This form is for approved SmartSlaps™ partners who have already received a kit and need additional materials. Every request is manually reviewed before resupply is approved and shipped.
A kit for a community mission.
This is a community fundraising starter kit: SmartSlaps and a short starting guide for your team. Share SmartSlaps to introduce your mission. Supporters scan the QR, open smartslaps.org/scan, choose a cause and donate. It is not a curriculum package or a personal sticker giveaway.
Choose the school, library, police, fire or EMS mission your team will support. Other community missions need Foundation review. An adult resident, parent, educator or community leader may organize a team for an eligible mission. Homeschool families and co-ops are welcome as adult-led teams for any community mission. We help you choose a recipient, set up your page and prepare your Guardian SmartSlaps. Personal expenses and personal-use sticker requests are not supported.
Your team sets its goal. We verify the recipient and donation route, help with setup, and confirm your SmartSlaps supply and shipping. Please submit once and use your reference when following up.

SmartSlaps™ reviews each request before shipping to make sure materials are going to real partners, real classrooms, and real community-serving organizations. This helps us protect inventory and keep the program strong.
